For us to be able to collect your passport you will need to supply us with a letter of authority (renewals only).
IMPORTANT NOTE REGARDING 24 AND 48 HOUR APPLICATIONS: We are sorry but lost, stolen, damaged and first time applications cannot use our 24/48hr service.
Just read and follow the 9 easy steps below and select a payment method that you prefer.
HOW TO PAY FOR YOUR NEW PASSPORT: We provide access to Paypal (you do not need a Paypal account to use this service), you can write a cheque to us or you can call just us and pay over the phone with your preferred card. We take payments from the following providers:
1. For us to help you get your passport it is easier if you supply us with your original birth certificate or a duplicate which can be obtained from the registry office where you were born.
Note: If you use a duplicate please enclose 2 utility bills from different companies (e.g. gas, electric bank statement).
2. If your name has changed since you applied for your last passport we will need proof of this change. An example of proof is if you have been married recently a marriage certificate can be used. If you have had a change of name a then a change of name deed will suffice.
IMPORTANT NOTE REGARDING LANGUAGE OF DOCUMENTS: All documents must be originals, in English and not photocopies. Documents in any other language except English must be translated by an official, Government approved translation school.
3. If you and both of your parents were born outside of the UK then we need a naturalisation certificate.
4. Have 2 identical passport photographs
5. Get a passport application form from your nearest post office and complete it using BLACK INK
6. Only fill out the following sections
- Adult first time lost damaged or stolen
- Personal Details
- If your passport has been lost or stolen you must complete section 3. You must also complete an LS01 form available from the post office and send with your application.
- If you were born after 31st December 1982 complete your Parent/Guardian details
- Signature of applicant must remain inside the white box
- Section 10 must be completed by the counter-signatory in their own handwriting.
7. One of the passport photographs must be signed by your counter-signatory with the following: ‘I certify that this a true likeness of (your name), signed (their name)’.
8. Pay for your passport service in 3 easy ways
i. Write a cheque to Passport and Visa Service Ltd and return it with your application form
ii. Or use our Paypal link below, selecting the service your require (you do not need to have Paypal account to use this service and it costs no more). Payment will be taken by 1st Premier Leisure on behalf of Passport and Visa Service.
iii. Or call us on 0844 357 9667 to make a payment over the phone with your debit or credit card.

9. Please send us:
- The completed form
- Your original birth certificate or duplicate
- Two utility bills if using a duplicate birth certificate
- Marriage certificate (if required)
- Change of name deed (if required)
- Naturalisation certificate (if required)
- Form LS01 (if required)
- Two signed photographs
- Cheque
DELIVERY NOTE: For our 24 and 48 our service you need to collect 2 Royal Mail special delivery envelopes. Address the first envelope to yourself where you want the new passport to be delivered.
Address the second envelope to passport and visa service and send it with the application.
Alternatively you could use Mail Boxes Etc a UK wide courier service.
