You will need to provide the following information and a payment:
One photocopy of your passport photograph page. You can scan this page and email it to us or just send it by special delivery if you are doing this via post.
A properly completed application form found here – Cuba Visa Application Form . You should fill this out and scan it so that you can send this with your other details via email or just send with your other details via post.
A copy of your travel confirmation. This can be a scanned copy emailed to us or a copy of an email from your travel agent otherwise you can send a coy with your other details by post.
TOP TIP: The best option, to save you time and hassle, is to send the scan of your passport page, your itinerary (scanned or an email from the agent) and the scanned application form as one attachment in an email and then pay online.
